机用根管锉在老年患者牙合创伤性根尖周炎临床治疗应用分析

  【摘要】　目的　观察 ProTaperfile 机用系统在老年牙合创伤性根尖周炎患者中根管治疗的效
果。方法　随机把需要进行治疗的牙合创伤性根尖周炎患者

125 例, 220 颗患牙随分为机用大

锥度

proTaper 镍钛锉治疗组（P 组 前牙 64 颗，后牙 42 颗）和手用不锈钢 K 锉（H 组 前牙

73 颗，后牙 41 颗）,P 组用 ProTaper 机用镍钛锉冠根向逐步深入法预备管,H 组用手用不锈
钢锉逐步后退法预备根管

,观察两组在根尖引流效果、调牙合效果、根管治疗的完善程度，治疗

操作时间，根尖周破坏的愈牙合程度，牙齿有无折裂的差异。结果　

P 组根尖周炎治疗效果

优于

H 组的预备效果, 具有差异性(

P＜0.025);两组的充填时间 P 组优于 H 组(P<0.01)。结论 

ProTaper 机用锉在老年牙合创伤性根尖周炎临床治疗中优于统手用不锈钢锉的临床治疗。

 [Abstract]　 Objection The goal observes ProTaperfile machine with the system the effect

proTaper nickel titanium file treatment group (P group of front tooth 64, latter tooth 42) and the 
hand  with  the  stainless  steel  K  file  (H  group  of  front  tooth  73,  latter  tooth  41),  P  group  with 
ProTaper  machine  with  the  nickel  titanium  file  crown  roots  to  the  thorough  method  preparation 
tube,  H  group  gradually  retrocedes  gradually  with  the  hand  with  the  stainless  steel  file  the  law 
preparation root canal, observes two groups in the root point drainage effect, the adjust occlusion 
effect, the root canal treatment perfect degree, the treatment operating time, the root tip destroys 
recovers the clutch degree, the difference which whether there is the tooth broken.Results P group 
of root point week inflammation treatment result surpasses H group of preparation effect, has the 
difference  (P<  0.025);  Two  group  of  backfill  time  P  group  surpasses  H  group  (P< 
0.01).Conclusion  ProTaper  machine  surpasses  the  series  with  the  file  in  the  old  age

 1 临床资料

1.1 一般资料 2009 年-2011 年在我院就诊的老年患者牙合创伤性根尖周炎患者 125 例,男、

女无分组

;年龄 60～85 岁;220 颗患牙(72 例患者中有 83 颗后牙；53 例患者中前牙 137 颗)均

按以下统一诊断指标收集

:

A 患者主诉有根尖周炎己及临床检查有根尖周炎的临床症状;
B 除牙合创伤因素外患牙无明显引起根尖周病的其它病因;
C 患牙均可查出明确的牙牙合创伤感因素。
1.2 口腔门诊检查包括： A 牙体、牙周、牙髓的临床情况检查，有无牙体发育异常，有

无牙列发育异常

;B 常规 X 线根尖片检查;C 患牙的牙牙合关系检查,包括咬牙牙合习惯、早接

触、牙牙合干扰、错牙牙合情况及其它牙牙合创伤因素的检查；

D 根尖周病的病变程度与范围。

随机把门诊患者分为实验组和对照组。实验组采用登氏博机用大锥度

proTaper 镍钛锉（P 组
