In researching various types of integration tools, I 

came upon this general report format.  I find it directly 

applicable as a performance measurement reporting 

vehicle.  Using one table for each CSF metric, this 

report focuses on purpose and results.

Consider using this format as a general reporting tool 

for any metric.  It explains the reason for the measure. 

 It presents the results as data and in graphical 

format.  And, lastly, it analyzes and interprets the 

results.  Done correctly, a collection of these schedules 

could effectively convey the fiscal health of an entity.
